1. How to Check the EA Form Slip on My Record

• To check the EA Form slip, the account owner can refer to the following steps:

Step 1: Click My Record

Step 2: Click Payment

Step 3: Click Yearly Statement

Step 4: All EA Form slips will be displayed in the following section

Step 5: To check the EA Form slip information, you can click the eye icon

2. How to Download the EA Form Slip on My Record

• To download the EA Form slip, the account owner can refer to the following steps:

Step 1: Click My Record

Step 2: Click Payment

Step 3: Click Yearly Statement

Step 4: Click Download on the desired EA Form slip
